Problem running Order API CGI

Can anyone tell me what kind of scenario might produce this result when
running a custom CGI with the Order API?

What operating system is your server, what language is your custom CGI
written in, and what is it's filename?

RedHat, PHP (yes it's configured to run on my server that way), and I tried
using both orderproc.php and orderproc.cgi
