Adding Forms

We would like to use ShopSite as a way to register for a 5K race we are sponsoring. The race requires additional information about the runner such as name, phone, address, email, age, t-shirt size, etc. and if possible, a digital signature for a liability waiver. ( The waiver is not required; we can deal with the waiver on the day of the race.) I can use the Ordering Options area on the product page to add additional text to the order page and ONE pull down menu, but I need more than that. And I need the additional input from the runner to be handeled the same was as the shipping information so that our ordering person can put things together for the race. Can anyone suggest a way to do this? Thanks!
